Murrieta Youth Soccer League












Risk Management Procedure
All *candidates MUST have the current information entered in CORIS database by the league Registrar.

*Definition of Candidate: ANY and ALL persons engaging in an administrative capacity within the YOUTH sports program. Including but not limited to Board Member, Team Parent, Head Coach, Assist.Coach and Referee.

STEPS:
1.) E-mail the Registrar and let her know that you're going through the Risk Management process. She'll need the following:
a. Name
b. Address
c. Drivers license with expiration date
d. DOB
e. Age group of team
f. User name & password - so you can have access to your file. Once your file is set up, you can access your account by going to calsouth.com, click on Youth Soccer Login to access sign in screen.


Attach to the e-mail above, your picture for your file.
2.) Set-up an appointment with Quick Scan (909) 841-8192 OR the Murrieta PD. (951)304-2677. Costs vary, so please ask the provider what they charge and type of payment they accept.

Please Complete, Print & Sign 3 copies of the following form (copy & paste link to your browser)

a.) Request for Livescan Form Request for Live Scan Form

As you complete the form, enter the following information pertaining to our league:
League Name: Murrieta Youth Soccer League, Team Name: Unless you know it, leave it blank.

Please enter your e-mail address & phone numbers.
3.) The following form must also be completed:

b.) Progam Administrator Registration Form -

Administrator Application (Please print and turn in 3 copies)

As You complete the form, enter the following information pertaining to our league:

SEASON: unless you know it, leave it blank
LEAGUE: MYSL
CLUB: will either be REC for Recreational or FC Murrieta for club soccer
TEAM ID Number: unless you knwo it, leave it blank

NOTE: You'll need to contact the MYSL Vice President if you plan to take your Livescan at one of MYSL's hosted Livescan sessions.
4.) Once you have completed Livescan, email the Registrar and she will make arrangements with you to drop off the above forms, she will also need your picture (headshot) for the database, please e-mail the picture to the Registrar.

Questions & Answers

Q: Is Risk Management mandatory if I am helping my child’s team?
A: Absolutely! Anyone working with youth MUST have Risk Management approval. There are NO exceptions!

Q: What is the difference between Risk Management and Live Scan?
A: Risk Management and Live Scan are one in the same. Risk Management is the procedure and Live Scan is the term used based on the digital imagery fingerprints are now taken.

Q: If I have taken the Risk Management for my work does it count?
A: Unfortunately NO. Like all new Jobs.. Risk Management is a process that must be taken anew per the organization or Job you are attaining.

Q: If I took the Live Scan with MYSL in the FALL season 2006 do I need to take it again?
A: No, once approved your live scan is good for a life time provided you took it in the FALL of 2006 or later.

Q: I think I have Live Scan approval, how can I check on that?
A: If you think you may be approved already, please verify with your league Registrar and he/she can let you know if you are good or will need to take it again.
