Risk Management Procedure
All *candidates MUST have the current information entered in CORIS database by the league Registrar.

*Definition of Candidate: ANY and ALL persons engaging in an administrative capacity within the YOUTH sports program. Including but not limited to Board Member, Team Parent, Head Coach, and Assist.Coach.

STEPS:
1.) E-mail the Registrar and let her know that you're going through the Risk Management process. She'll need the following:
a. Name
b. Address
c. Drivers license with expiration date
d. DOB
2.) Once the registrar has entered your information into CYSA Coris database and she has confirmed, you'll need to set-up an appointment with Quick Scan (909) 841-8192 OR the Murrieta PD. (951)304-2677.

NOTE: You'll need to contact the MYSL Vice President if you plan to take your Live Scan at one of MYSL's hosted Live Scan sessions.

3.) You'll need to bring the following completed form with you: Request for Live Scan Form (PRINT 3 COPIES)
4.) Once you have made the appointment, email the Registrar and she will make arrangements with you to turn in one form to her within 48 hours and she will also take your picture for the database.

Costs may very so please ask the provider what they charge and in what form of payment they accept.

Questions & Answers

Q: Is Risk Management mandatory if I am helping my child’s team?
A: Absolutely! Anyone working with youth MUST have Risk Management approval. There are NO exceptions!

Q: What is the difference between Risk Management and Live Scan?
A: Risk Management and Live Scan are one in the same. Risk Management is the procedure and Live Scan is the term used based on the digital imagery fingerprints are now taken.

Q: If I have taken the Risk Management for my work does it count?
A: Unfortunately NO. Like all new Jobs.. Risk Management is a process that must be taken anew per the organization or Job you are attaining.

Q: If I took the Live Scan with MYSL in the FALL season 2006 do I need to take it again?
A: No, once approved your live scan is good for a life time provided you took it in the FALL of 2006 or later.

Q: I think I have Live Scan approval, how can I check on that?
A: If you think you may be approved already, please verify with your league Registrar and he/she can let you know if you are good or will need to take it again.
